Medical Practitioners

If you are a Medical Practitioner who would like to refer a patient, please email the referral to referral@aes.care

Alternatively, please fax the referral to the relevant clinic:

Please include the following in the referral:

1. Ophthalmologist to whom you are referring the patient
2. Patient first name
3. Patient surname
4. Patient date of birth
5. Patient street address
6. Patient suburb and postcode
7. Patient mobile phone
8. Patient home phone number or alternative contact number
9. Reason for referral
10. Urgency of referral (within 1 week / within 1 month / etc.)
11. Any treatments to date regarding eye(s)
12. Patient medical history (including family history)
13. Previous operations and procedures
14. Allergies
15. List of current medications (including over-the-counter, nutritional supplements or herbal preparations)
16. Previous operative notes on any eye procedures
17. Investigations, such as radiology reports, blood tests, previous results from OCT,  visual field tests, angiograms, lasers, etc.
18. Your contact details, provider number and centre / practice location

A member of our staff will contact the patient directly, once the referral has been triaged, to schedule the appointment.

Optometrist or Ophthalmologists

Optometrist and Ophthalmologists may email referrals using Oculo's secure web-based platform.  Click here to refer to us via Oculo.

If you are not currently registered with Oculo, please register online via their website (please review their fees).
